1. A description of your programs (please provide information on program length and cost.)


2. A sample program including; a description, a breakdown of the specific curriculum fit, and pre and post activities given to program participants.


3. Three user evaluations or letters of support from teachers or community leaders participating in your program.

4. Your organizations program evaluation strategy.


5. With regards to the safety and security of program participants please elaborate on some of your organizations risk management practices including; first aide, emergency procedures, liability coverage, and selection of volunteers if applicable.
