HookedonCoons Pet Adoption Questionnaire

Thank you for your interest in one of my Maine Coon kittens or retired adult cat. I make every effort to match the kittens or cats with the best family or individual for that particular kitten or cat. In order to determine the best match I ask that you please complete the following questionnaire. All responses will remain COMPLETELY CONFIDENTIAL. Please return this document as an attachment to an email to hukncoon@telus.net or by snail mail contact me for my address.

State your name, address, phone numbers and e-mail address (es).

What is the best time and method of contacting you? What is the second best time and method of contacting you?

Why are you interested in a Maine Coon? Were you looking for a pet/companion, show alter, or a breeding cat? List everything that applies.

When would you like to get a cat?

Would you prefer a male or a female or does the sex of the cat not make a difference?

Would you accept a cat that is different from the sex that you requested if that was all that was available at the time kittens are available?

What color/pattern cat were you interested in? Would you accept a different color/pattern?

Would you like a kitten, retired breeder, or a potential show cat?

Is the cat to be a pet for you or a gift for someone else?

How much time would the cat spend alone each day?

Where would the cat spend its day? Where would it spend its night?

Who would be the primary caretaker of the cat?

Is there any one in your house that does not want a cat?

Describe any additional requirements you have in a Maine Coon kitten.

If you belong to a home or condominium association, does your association allow pets?

List all the pets in your home along with a description of the pet, their personality, and their age.

Have you ever owned a cat before? (if so, how long and what happened to the cat)

Do you have cats living with you now? If so, how many? What breed? How old? Are they altered? Are they de-clawed? Are they in good health? What kind of cat food do you feed? Are they current with their vaccinations? Are they indoors only? If kept indoors, how often do you clean litter boxes?

Do you currently have a veterinarian? May we provide you with a veterinarian reference form for you to have your vet complete and forward to us?

How did you hear about us? Cat show, newspaper ad, referred by someone (who?).

Feel free to add any other comments that you'd like to share about yourself or why you think you could provide the ideal home for one (or more) of my kittens. I cannot guarantee a cat/kitten to everyone that fills out and responds to this questionnaire. It is simply a form designed to help me evaluate potential owners and their environments for the placement of my cats/kittens

If you are interested in one of my kittens you may copy and paste this questionnaire on to your word processing program and fill in the questions from there.
