Agreement to provide personal financial information:

 The words "you," your" and "yours" mean each person submitting the personal information. The words "we," "us," "our" and "ours" as used below refer to us, Good Cents Co., operated by Dave and Laurie Lee, registered as Good Cents Debt Management, and to the institutions selected to receive your personal information and any assignees as provided below.

 You authorize us to submit this personal information and any other information submitted in connection with the proposed transaction, through Good Cents Co., to the institutions disclosed to you by us, Good Cents Co. This information will be reviewed by such institutions on behalf of themselves and us, Good Cents Co..

 You authorize that Good Cents Co. obtain a consumer credit report periodically from one or more consumer reporting agencies (credit bureaus) in connection with the proposed transaction and any update, renewal, modification or extension of that transaction and to determine if there has been a change in your consumer credit report.  If you ask, you will be told whether a credit report was requested, and if so, the name and address of any credit bureau from which we obtained your credit report

 You agree that we may verify your, pay, assets and debts, residence, and that anyone receiving a copy of the information, with this disclosure attached, is authorized to provide us with such information. You further authorize us to gather whatever financial history we consider necessary and appropriate in evaluating this information and any other personal information submitted in connection with the proposed transaction.

 We may keep this personal information and any other personal information submitted to us, by you, and information about you whether or not the services are accepted or in use. You certify that the information submitted to us, is true and complete. You understand that false statements may subject you to criminal penalties.
