By requesting a consultation I am in agreement to the following:

• I know this consultation is complimentary to any counseling/treatment I may be requiring and not a replacement for it.
• I know this consultation is based solely on the education, experience, and research of Sandi Harpham and not a specific theory, or backed by a registered association.
• I know this consultation is based on balancing the physical, mental, emotional, and spiritual aspects of myself.
• I assume full responsibility for my participation in, use of, and results from the information received in this consultation.
• I am familiar with the material in Living On Purpose: Life Beyond Mediocrity, or similar material.
  
• I believe there is an upward spiral to life and I am looking for assistance in remembering it.
