Respect for your time:  Our primary goal is to assist you in attaining and maintaining optimal oral health.  Therefore, your appointment time is reserved exclusively for you.  We will respect your time and trust you will respect ours by arriving on time for your appointments.  If it is necessary to cancel an appointment, we ask that you give us as much notice as possible so that your previously reserved time can be made available to another patient.

Our office requires at least 48 hours notice to reschedule your appointment unless you have become sick or have a family emergency Inconsiderate cancellations may be subject to a fee based on the time and procedure cancelled.  Patients who consistently cancel or miss appointments without notice will not be rescheduled.

When you arrive for your appointment, please have you health and insurance information (if applicable) available.  If you happen to be transferring from a previous office, you can facilitate the patient examination process by having your records forwarded to our office beforehand - in particular, x-ray images & periodontal charting.  If you wish to transfer records to our office, please click here for a copy of the records transfer authorization form.
